Durham Region

Two models for assessment and diagnosis of FASD are available in Durham Region for children under the age of 18. Both incorporate the University of Washington training and comply with the Canadian FASD Diagnostic Guidelines (February 2016).

Grandview Children’s Centre for preschool children < 5 years of age:
Grandview offers programs and services to children and young adults with physical and communication disabilities who live in the Durham Region. A family-centered approach emphasizes collaboration with parents, community agencies, educators and physicians. Medical specialists offer consultation, assessment and diagnosis services – including FASD, and advocate for the provision of medical services as needed. A medical referral is required; please indicate that the referral is an FASD query.

Children who are school-aged (5+years of age) living in Durham Region are assessed through a Virtual Diagnostic Team led by a developmental paediatrician in private practice. Referrals are streamed through a local agency, Resources for Exceptional Children and Youth, that is responsible for coordination of the file information and linking parents/caregivers with the appropriate clinical discipline to advance the diagnostic process (some clinical fees may apply). A medical referral is required.

The CHEO FASD Assessment Clinic provides a multi-disciplinary diagnostic assessment for children, youth and adults with suspected FASD. The assessment team is comprised of a Social Worker, Psychologist, Psychometrist, Occupational Therapist, Speech Language Pathologist and Physicians.

Referrals
Eligibility Criteria for referrals:

  • The individual must have confirmed or suspected exposure to alcohol before birth
  • The individual must be 4 years of age or older – please note we will also accept referrals for adults
  • The individual must have significant developmental, behavioural, and/or mental health concerns
  • The individual must live within the Champlain LHIN (Ottawa, Renfrew County, Prescott and Russell, North Lanark/North Grenville and Stormont, Dundas &amp; Glengarry)
  • If a child protection agency or adoption agency was involved with the individual, all available records must be provided with the referral.

A completed psychological assessment is no longer required prior to referral. Our team will complete this assessment if one has not already been done. Referrals must be made by the legal guardian or by a physician. All referrals are made through the CHEO Access Team.

Guelph-Wellington

Fetal Alcohol Spectrum Disorders Diagnostic Team

Referrals will be accepted when there is confirmed maternal alcohol use and when behavioural issues are having an impact on the children’s leaning and day to day functioning.

Our Team consists of a Speech Language Pathologist, Occupational Therapist and Paediatric Neuropsychologist. Each member of the multidisciplinary team has several years of experience (from 2 to over 30 years) in FASD assessment and diagnosis and each provides a written report to the family of their particular assessment. The neuropsychological report integrates all of the conclusions from the other team members with the results of the neuropsychological assessment to provide one comprehensive diagnostic report, which is provided to the family at the end of the process.

Receipts are provided upon each payment. Families are often able to submit receipts to their Third Party Insurance providers for full or partial reimbursement or may be able to apply the costs to the Child Disability Tax Credit or other government sponsored programme. We provide full diagnostic and assessment services for children, teens, and adults who are living in Ontario. We use the Canadian Diagnostic Guidelines for the diagnosis and assessment of individuals with FASD. Our multi-disciplinary team consists of a Clinical Neuropsychologist, Speech-Language Pathologists, and an Occupational Therapist, each with experience in FASD assessment and diagnosis. Families/referral parties will receive one comprehensive integrated report that includes all information from each team member. The evaluation includes review of all relevant medical/academic records, an Intake Interview with the Pediatric Neuropsychologist, separate but integrated assessments by all team members, feedback of assessment results to family and school personnel (either together or separately depending on client preference), and the completion of one comprehensive integrative report that will be provided to families/referral parties.

Receipts are provided upon payment. Assessments may be flat rate or hourly rate, depending on the individual case. We try very hard to work with families to ensure a comprehensive yet accessible service. Families are often able to submit receipts to Extended Health Benefits (if they have coverage for these services under their employee benefit plan) for some reimbursement, although not always in full. Kingston

KidsInclusive Centre for Child & Youth Development – Kingston Health Sciences Centre

KidsInclusive provides rehabilitation and support services for children and youth with physical, neurological and/or developmental challenges in the Kingston, Frontenac, Lennox and Addington counties as well as communities in Lanark, Leeds and Grenville counties. Some of our specialty services including FASD assessments support children and youth in Prince Edward and Hastings counties. We have capacity (although limited) to provide assessments for possible Fetal Alcohol Spectrum Disorder (FASD) to children 12 years and younger, as we have the expertise of a Developmental Paediatrician and Psychologist on staff with FASD training. Each referral is reviewed carefully to ensure there is confirmation of Prenatal Alcohol Exposure (as per the Canadian FASD Guidelines) and documented global developmental delays (i.e., speech, language or motor assessment reports).  If there is no confirmation of prenatal alcohol exposure, referrals are not accepted. We can help guide you through the process to access supports and services that may be available to you. There is not one consistent pathway for assessment at KidsInclusive as each child or youth and their family have a unique history and experience that will determine the process for their assessment. Some children will receive a multi-disciplinary assessment while others will be assessed by the Developmental Paediatrician or Psychologist or by both.

We offer FASD Diagnostic Services for children up to 18 years of age living in the London Region. Our team consists of Developmental Pediatricians, Occupational Therapists Psychologists, Speech-Language Pathologists, and a Clinic Coordinator. All of the clinicians have significant expertise in the area of FASD, and have been trained through the University of Washington – The 4-Digit Diagnostic Code and our practices comply with the Canadian FASD Diagnostic Guidelines (March 2005).  We provide full assessments, recommendations, and assistance with referral to appropriate community programs.

The Northwestern Ontario FASD Diagnostic Clinic conducts 11 clinics annually for children, youth and adults with confirmed prenatal alcohol exposure.  Clinics are held in Sioux Lookout, Fort Frances and Kenora, providing flexibility to receive assessments and diagnosis closer to home.   Our assessment process includes family preparation, speech-language, occupational therapy, neuro-psychological and physician assessments. Families receive a summary report with recommendations, and are eligible for follow up with an FASD Worker. Families can also access support from the team’s Bicultural Clinician. The Northwestern Ontario FASD Diagnostic Clinic, supported by FIREFLY, is a collaborative project between 11 partnering agencies:  William W. The Peel Region FASD Clinical Service is a virtual team with a developmental paediatrician, a paediatrician, a psychologist, a psychometrist, an occupational therapist, a speech and language pathologists, three social workers and a coordinator who provide assessments at their respective agencies. At the end of the assessment process, members of the clinical team come together to meet with the family and discuss the diagnosis. Reports are written and mailed to the family and a social worker follows up to discuss the reports and recommendations.

In order to be eligible for assessment, children must be between the ages of birth and six years of age, live in Peel Region and have confirmed prenatal alcohol exposure and developmental concerns. Referrals are open and can be made from any source.

NEO Kids FASD Clinic at Health Sciences North is a collaboration with the Northeastern Aboriginal FASD Education Partnership (NEAP), providing full FASD assessment services for children and youth 18 years of age and younger in Northeastern Ontario.

Referral criteria includes:  18 years of age or younger; confirmation of maternal alcohol exposure.

The Mothercraft/Breaking the Cycle (BTC) Pediatric/FASD Diagnostic Clinic is an integrated component of BTC and includes pre- and post- assessment services. Access to the Clinic is limited to children and mothers who are clients of  BTC, a.  a CAPC/CPNP program serving pregnant and/or parenting women with substance use problems, and their young children (birth – 6 years). BTC is an early intervention program designed to reduce the risk and enhance the development of substance-exposed children by addressing maternal substance use issues and the mother-child relationship. Since 1995, BTC has delivered a comprehensive, cross-sectoral, maternal-child program to provide a range of integrated services through a single-access model, with home visitation and pregnancy outreach components. BTC is delivered through a formal service partnership between Mothercraft and  Toronto Public Health, the Hospital for Sick Children-Infant Mental Health Promotion, St. Joseph’s Health Centre, Toronto Western Hospital – Addictions and Mental Health, the Children’s Aid Society of Toronto, the Catholic Children’s Aid Society, St. Michael’s Hospital and the Ministry of Community Safety and Corrections

All BTC children for whom prenatal alcohol exposure is confirmed are referred for an FASD assessment and for regular developmental follow-up. Full physical and neuro-developmental follow-up occurs on an annual basis (or more frequently depending on clinic team recommendations)..  All children receive assessment-based early intervention services. The FASD diagnostic team at BTC includes: Program Coordinator, Developmental Pediatrician, Clinical Developmental Psychologist, Case Manager/Addiction Counsellors, and Early Childhood Interventionists.

This clinic uses a multidisciplinary approach that provides diagnostic services, a plan of care for each identified individual, and co-ordination of community referrals as needed. The clinic assesses individuals of all ages with suspected FASD. Infants who have been identified because of prenatal exposure in high- risk mothers are entered into the clinic from birth.

 The Surrey Place FASD diagnostic clinic provides services to individuals with prenatal alcohol exposure and developmental concerns of all ages, including adults. The team offers diagnostic, educational, and advocacy services. If an individual assessed is found to be eligible for developmental services, treatment options such as counselling or case management may also be offered. Our multi-disciplinary children’s diagnostic team consists of a Paediatrician, Speech Language Pathologist, Occupational Therapist, Audiologist, Clinical Psychologist, Behaviour Therapist and Service Coordinator. Our adult diagnostic team consists of a Clinic Coordinator, Nurse Specialist, Speech Language Pathologist, Clinical Psychologist, and Behaviour Therapist.  Our catchment area is the city of Toronto. Individuals residing outside the city of Toronto may be considered on a case-by-case basis and offered services for a fee.

Our team consists of a Paediatrician, Psychologists, Occupational Therapists, Speech Language Pathologists, and Clinic Coordinator. All of the clinicians have expertise in the area of FASD and have completed the University of Washington 4-Digit Diagnostic Code training.
